Apalachee Center’s Florida Assertive Community Treatment (FACT) is designed to provide comprehensive community-based help to individuals with serious mental health challenges who need more than regular case management or medication services can provide.
FACT provides 24/7 wraparound mental health services based on a nationally recognized, evidence-based treatment model, Program of Assertive Community Treatment (PACT). FACT provides intensive treatment, rehabilitation and support services to clients in their homes, on the job and in social settings and helps get individual’s basic needs met, enhances their quality of life, supports independent living, increases community involvement and activity and assists clients in obtaining employment, educational opportunities and volunteer experiences.
Our Team
Apalachee Center’s FACT team is led by a licensed clinician and an advanced practice nurse practitioner (APRN), both with extensive experience and training in working with individuals with severe mental illness and co-occurring disorders.

Services
Apalachee Center’s FACT program offers a wide range of integrated services to help our clients
- Psychiatric evaluations & medication management
- Our advanced practice registered nurses (APRNs) meet with clients monthly to assess, diagnose and treat symptoms with medications. They also monitor the non-psychiatric medications and work closely with the client’s primary care physician.
- Case management
- With a team of qualified care managers, FACT maintains a low client-to-staff ratio to better serve our clients.
- Nursing
- Registered nurses (RNs) manage and administer medications, including injections, and screen for other health problems while coordinating with other providers.
- Peer support
- Peer specialists use their lived experiences to mentor and support others in recovery with mental health and/or substance use issues. By listening, relating, caring and providing resources, our trained and state-certified peer specialists help our clients reach their recovery goals.
- Mental health and substance use disorder treatment
- Our counselors and therapists provide integrated dual diagnosis treatment, therapy, outreach and relapse prevention.
- Vocational rehabilitation services
- A dedicated vocational rehabilitation specialist provides supported employment services including job development, job placement and job coaching.
Eligibility
Individuals who qualify for FACT:
- Have been diagnosed with mental illness and/or substance use disorder, or co-occurring disorders and are experiencing significant functional changes
- Have a history of frequent or lengthy psychiatric hospitalizations with their current level of outpatient treatment
- Have difficulty taking care of basic needs due to mental health and/or substance use or co-occurring disorders
- Are willing to work with the FACT team
