Homeless Projects

We realize how difficult it can be to get back on your feet when you struggle with mental health or substance use and other life challenges.

Apalachee Center’s Homeless Project is an outpatient treatment program providing services to individuals who are experiencing homelessness, have no insurance, and have been diagnosed with a mental illness/co-occurring disorders. The purpose of the Homeless Project is to provide supportive services, such as jobs and housing, alongside of psychiatric services like evaluations, medications, and primary care services. Our team works in the community to provide better access to care and we want to help our clients achieve their goals of gaining independence and a better quality of life.

 

What Is Homelessness?

According to the Public Health Service (PHS) Act;

  • An individual experiencing homelessness is defined as “an individual who lacks housing (without regard to whether the individual is a member of a family), including an individual whose primary residence during the night is a supervised public or private facility that provides temporary living accommodations (i.e. shelters, streets, car etc.), and an individual who is a resident in transitional housing.”
  • Imminent Risk of Becoming Homeless is defined as “living in a condemned building without a place to move, having arrears in rent/utility payments, receiving an eviction notice without a place to move, living in temporary or transitional housing that carries time limits, and/or being discharged from a health care or criminal justice institution without a place to live.”

 

SERVICES

  • Psychiatric Evaluations / Medication Management

Our APRNs (Advance Practice Registered Nurse) meet with clients to assess, diagnose, and treat symptoms through medications. They also monitor the non-psychiatric medications and work closely with the primary care physicians.

 

  • Case Management

Our team develops case plans to deliver community services to coordinate evaluation, treatment, housing, and resources, tailored to each individual’s needs and preferences.

 

  • Outreach

Our team works in the community including street outreach, meeting people experiencing long-term or multiple episodes of homelessness and developing the relationships necessary for supporting transition to affordable housing and needed treatment. We work to bridge the gap between individuals and available services and resources.

 

  • Financial assistance and Supportive Housing Services

We provide services to assist people in choosing, obtaining, and retaining housing in a regular, integrated setting in the community.

 

  • SSI/SSDI Outreach, access and recovery (soar) Services

SOAR is a national program that increases access to Social Security/Disability for eligible adults and children who are experiencing or at risk of homelessness and have a serious mental illness, medical impairment, and/or a co-occurring substance use disorder. Our trained staff will work with our clients and assist in the application/appeal processes.
